Care Coordination is a community-based home care program administered by professional case managers which promotes independent living for persons age 60 and over.
Independent Living
Services include assistance with activities of daily living, home-delivered meals, minor residential repair, health maintenance (Dental, vision, hearing, and medication), emergency transportation and in-home service arrangement.
Affordable Professional Dental Care
TCOG is proactively involved in an innovated approach to provide a much-needed affordable dental care program called Smiles for Texoma (SFT)
Smiles for Texoma assists with the continually escalating dental needs of the low-income, disabled, frail, and elderly citizens of the Texoma area.
Through SFT, participating Texoma American Dental Association members agree to offer treatment at a reduction in normal rates, with the exception of lab fees, which are charged in full. Our goal is to help those seniors who would otherwise have no means of affording professional dental care.
